In this Clinical Update series, PSS Hospital Chapter shares insights from pharmacists in the ambulatory care setting on the shift from face-to-face clinic consults to teleconsultation during this period. International Society of Cardiovascular Pharmacotherapy (ISCP) is a Global organization, affiliated to the World Heart Federation, whose mission is to promote and facilitate strategies to improve cardiovascular health through rational pharmacotherapy. The Practice & Governance workgroup have launched a new series called “CLAP in 60 secs”, which is a 1 minute sharing of cardiovascular pharmacotherapy issues. (CLAP = CataLysing reAl-world cardiovascular Pharmacotherapy) Here's the latest release of “CLAP in 60 secs” on the dosing of OAC in renal impairment with a downloadable cheat sheet on OAC dosing in AF and VTE given by Dr Doreen Tan.
