Clinical Pharmacy Practice Guidelines Print E-mail

A new set of Clinical Pharmacy Guidelines on Management of Chronic Heart Failure has been released in Oct 2007.

The objectives of these guidelines are to provide an up-to-date and practical approach for the management of a patient with chronic heart failure and to assist pharmacists in the pharmacological and non-pharmacological management of chronic heart failure patients.

These guidelines will support pharmacists practising in heart failure clinics in an outpatient setting, titrating mainly angiotensin converting enzyme inhibitors, angiotensin II receptor blockers and beta-blockers to target doses after these have been first prescribed by a physician. It can also be used by other healthcare professionals concerned with the care of patients with chronic heart failure.

For these and an earlier guideline on Anticoagulation- Warfarin, please refer MOH Health Professionals Portal.
